Membership
Personalized Gynecology for Midlife Women
A physician-led gynecology practice dedicated to individualized care for women in midlife.
The practice is intentionally kept small so there is time to listen carefully, understand your history, and provide continuity with the same physician who comes to know you and your goals.
Care is delivered through a direct-pay membership model which allows for longer visits, ongoing guidance, and coordinated care with Dr. Bazán. Visits may take place virtually or in person, depending on what best supports your care.
Continuity of Care
You will always see Dr. Bazán, every visit.
Initial Consultation | Enrollment – $495
Your first visit is a comprehensive virtual consultation that includes:
- 45–60 minutes of face-to-face time
- Detailed review of medical and gynecologic history
- Review of prior labs and records (when available)
- Exploration of personalized treatment and management options
- Development of an initial care plan, prescriptions, or referrals
The initial consultation is billed as an enrollment fee, reflecting your entry into the practice and the development of your personalized care plan.
Monthly Membership
$175/month
- Visits included to support ongoing, individualized care
- 6-month minimum commitment | 30 days notice for termination after initial term
What to Expect
- Care is designed to be thoughtful, individualized, and responsive to your needs over time.
- Most patients are seen periodically based on clinical needs, whether for symptom management, treatment follow-up, or preventive care.
- Some phases of care may require more frequent visits, while others may be more spaced out.
- Secure messaging is available for brief, non-urgent questions between visits. Responses typically within 1–2 business days
- More complex concerns are best addressed during a scheduled visit to allow for appropriate evaluation and care planning.
This model allows for continuity, flexibility, and the time needed to provide care that evolves with you.
Annual membership is available with a 10% savings when paid in full.
